Authorization: I/We authorize All Valley Hose & Industrial Supply to investigate my/our credit worthiness for the purposes of purchasing goods and services from All Valley Hose on credit. I/We authorize all companies and banks listed on this credit application to provide any information reqested by All Valley Hose in their efforts to establish my/our credit worthiness.

Terms: Invoices are due upon receipt of invoice. For accounts in good standing, invoices may be paid within 30 days of the date purchase. Invoices not paid by the due date will be assessed a $15.00 late fee for each month payment is not received. Accounts with unpaid invoices over 30 days will automatically be shipped on a C.O.D. or prepaid basis until satisfactory credit with All Valley Hose & Industrial Supply has been restored.

I/We agree that all accounts or monies due to All Valley Hose & Industrial Supply shall be paid in accordance with the credit terms stated above and agree to pay all reasonable costs of collection, in addition to any court costs and/or attorney fees incurred on behalf of All Valley Hose & Industrial Supply.

Guaranty: I/We do hereby personally guarantee payment, as individual(s), of any indebtedness incurred by virtue of any and all credit extended in accordance with the above agreement and all of its terms and conditions.
